Patterns of Loss

There’s a lot of talk about the Norwood scale in hair restoration circles these days. The Norwood scale is a good mechanism for determining the extent of a patient's hair loss. It allows both patient and doctor to anticipate the progression of loss. This is critical for proper planning in performing a natural and undetectable restoration.


But it doesn't identify the areas that men and women lose their hair. As a physician that has specialized ONLY in hair restoration for nearly 16 years, I have found it important to help my patients understand where they will be losing hair, the progression it will take and how this will impact their decision for various forms of medical or surgical treatment.


There are five primary patterns of hair loss for men. These are not mutually exclusive but rather should serve as a rough guide for the areas of loss and the typical patterns the recession takes.


The "A" Pattern

A-Pattern or anterior pattern of hair loss is the most common. It starts in the corners or temples in the late teens or early twenties and slowly works its way from front to back. Most men have some temple recession and it is considered normal. Women have rounded temples giving them an oval face, rather than a square face like men.


"V" Pattern

V-Pattern or vertex pattern or (crown) is less common than A-pattern. These men have less temple recession. Their loss starts in the center of the swirl & slowly spans outward. The expansion of the circle may or may not reach the front or temples, completing the horseshoe shape.


The "A-V" Pattern

A-V Pattern, these men show early signs of hair loss both in the temples and the vertex area at the same time. The A-V pattern type often go on to have the most complete hair loss resulting in class 6 or even 7. With hair loss going both from front to back and back to front. This results in the last hair loss is the area between the ears called the bridge.


"F" Pattern

F-Pattern or forelock, in this pattern the men often do not realize the extent of their loss. They are often in denial. The temple recession widens first then expands across the middle of the head, bridging across leaving an island or tuff of hair in the front called a forelock. These vary in size. These forelocks are resistant to loss & often remain until the 40’s or 50’s although the hair behind it is gone. Eventually this goes too.


"D" Pattern

D-Pattern or diffuse hair loss pattern. This is found more commonly in women.

However, many men have this as well. There is little or no temple recession. The hairline is stable, but their hair slowly gets thinner. Not only are there less hairs per square inch, but each hair shaft is thinner, we call this miniaturization.


Female Hair Los Patterns

The female hair loss pattern is a diffuse pattern starting primarily in the central-frontal area while keeping the temple and original hairline intact. The female often notices hair loss first by a widening of the part that spreads and thins out.


In all the hair loss patterns, the earlier you show hair loss the greater your loss will be, since it is a natural progressive condition.
