Folliculitis
While it may be a mouthful, folliculits could ruin your day
if not treated by a physician. This disease often goes unrecognized
but can be defined by various infections that involve the
hair follicle. It is normally limited to superficial areas
of the hair follicle but may progress to involve the deep
follicle. These are called furuncles (boils), which can form
a group called a carbuncle.
Folliculitis may affect any part of the body that is a "hair-bearing
area," but most commonly appears in the axillae, pubis,
thigh or scalp. It can occur by a break in the integrity of
the skin surface, which may become impaired or broken by irritation,
pressure, friction, scratching, insect bites, or shaving.
This provides a portal of entry.
By far the most common organism involved in folliculitis is
Staphylococcus aureus, but many other organisms can be involved,
including fungus and herpes virus. Factors predisposing to
folliculitis include, diabetes, obesity, malnutrition, immunodeficiency
problems, such as AIDS, and alcoholism. Chronic, resistant
or recurrent folliculitis is usually caused by autoinoculation
(self inoculation) or infra-familial spread, most commonly
from the nose. The diagnosis of superficial folliculitis is
based on finding a pustule (small area of skin elevation containing
pus) often pierced by a hair. The hair follicle is usually
not lost but can be if the folliculitis is chronic. This can
also result in scaring.
Treatment of folliculitis is typically accomplished by thorough
cleansing of the affected area with antibacterial soap three
times a day. A warm, wet compress should be applied for 20
to 30 minutes three to four times a day, which is usually
sufficient in mild superficial cases. Oral antibiotics can
be prescribed with various lengths of time, up to three months.
For deep folliculitis, i.e. boils and carbuncles, an M.D.
should be consulted for incision and drainage and packing
with gauze for one to two days along with a course of oral
antibiotics.
I wrote this article because I am sometimes asked about this
disease and thought I would take this opportunity to share
with you the specifics. If you or anyone you know is experiencing
