The Economics of Hair
The art of hair transplantation is comprised of several disciplines.
First are the surgical aspects. These relate to the way the
grafts are harvested from the donor area and then transplanted
into the receptor sites. Second are aesthetic disciplines.
These relate to the type of hair that is selected as well
as the depth, angle and direction for which the receptor sites
are created. Finally there is the discipline of hair economics.
This is the proper planning for available supply of hair in
the donor area as it relates to the required demand in the
receptor area.
The Supply
The supply of permanent hair is a horseshoe-shaped area that
starts just in front of the ears and dips down as a U-shape
in the middle of the back of the neck. In men that are a class
5 or 6, it appears to dip down to the top of the ears, giving
about two to three inches of transplantable hair. Men who
are class 2, 3 or 4, in their 20s, 30s or 40s,
may dip down to the class 6 level as they age.
To get a better idea of how far your donor area will dip.
Simply wet your hair, look in a mirror and use a second mirror
to view the top and back of your head. You will see a demarcation
line between your hair and skin near the crown of your head.
There is another line, maybe fainter, one inch or so below
that U-shaped line showing hairs that are thinning or miniaturizing.
These hairs should never be transplanted since they will be
lost someday.
The supply of donor hair can be calculated by multiplying
the width by length. The length of the donor area generally
is 10 to 13 inches long, depending on the size of your head.
The width or height of the donor area depends on the amount
of hair that you will eventually lose. The width of this area
is also dependent on the flexibility of the scalp. Scalp flexibility
varies not only by individual, but also in the different areas
of the scalp. The scalp is most flexible and the thickest
in the middle of your neck area but thins and becomes tighter
as you get behind the ears. Reach around to the back of your
head and feel this for yourself. Scalp flexibility helps us
to determine the width of donor area that will be removed
from the scalp.
Meeting the Demand
The biggest challenge and irony to hair loss is that ultimately
the less of a supply you have, the more you need. This is
evident when looking at a Class 6 hair loss pattern. It is
also true that the more hair you have, the less hair you need
to be transplanted. These are those Class 2 guys that we all
envy.
An additional factor in meeting demand is the number of available
hairs per square inch in the donor area. We call this density.
Hair is the most dense, or thickest, in the nap and mid area
of the neck and thins as it gets above the ears. Other variations
depend on your age, type of hair and texture of your hair.
While most people think of hair in terms of length and numbers
of hairs, the fact is each hair also has a volume. If you
double the radius of the shaft, it will quadruple your volume,
so people with fine hair have less hair volume than people
with thick hair. As hair ages, it miniaturizes, and the shaft
becomes finer. So hair loss is not only a matter of less hair
per square inch, but also each hair becoming thinner.
Balancing Your Hair Economy
In its most simplified form, hair restoration surgery is a
re-distribution of the hair from the area where its greatest
supply is located (at the back of the head) to the area of
greatest demand (at the front and top of the head). Generally
for the first hair transplant, a strip of donor can be taken
that is half an inch wide and 3/8 of an inch wide over the
ears. The more grafts you need, the larger the area that needs
to be removed.
With the second or third procedure, since the scalp is less
flexible, it may be difficult to remove the same width of
donor, which means less hair can be harvested on the second
or third procedure. The area to be removed becomes more and
more limited with each procedure. This is why it is important
to have an experienced physician who will take into consideration
not only your current hair loss situation, but what it will
be in 15 to 20 or even 30 years from now.
Planning for the variables of supply, demand, flexibility,
density and future hair loss is critical in maximizing the
short and long-term economics of your hair loss.
